This resource is inspired by and adapted from the ADHD medication diary developed by the Canadian ADHD Resource Alliance (CADDRA).
 
As neurodivergent folks, there may be times when our doctors may prescribe us with medications, such as antidepressants, mood stabilisers, psychostimulants, etc. It can be hard to keep track of the pros and cons that we're experiencing from the medication (especially if we're taking multiple different medications) and accurately communicate them at our next doctor's appointment.
 
If you can relate to this, this may be a helpful resourcs to help you clearly log your experiences with your medications, and either show it to your doctor or to refer to your log while communicating with your doctor at your next doctor's appointment or medication review. Page 3 has some empty rows for you to fill in, if you're experiencing a different side effect no listed in the template.
 
The idea is that you can fill one of these out whenever:
You have been prescribed a new medication, or 
You've been asked to change the dosage of your current medication (either increase or decrease), or
There has been no change in your medications, but you've experienced a sudden and significant change in your experience with the medication (either because the medication stopped doing what it's supposed to do for you, or you start experiencing side effects you didn't have before).
